Weight Loss / PCOS GLP-1 Membership Terms & Conditions

By enrolling in the Weight Loss/ PCOS Program, you agree to the following:


Minimum Commitment

All memberships require a minimum 4-month commitment. Memberships cannot be canceled, paused, or terminated during this period.


Medication & Financial Responsibility

Membership fees include medication, medical oversight, and program services.
Once medication is ordered, processed, shipped, or dispensed, the member is fully responsible for associated costs, regardless of usage or outcome.


Cancellation Policy

After the 4-month commitment:

  • A 30-day written notice is required 
  • Billing continues during the notice period 

Cancellation is not valid until confirmed by Blossom Medical Associates 


Required Cancellation Process

To initiate a cancellation request, members must:

  • Submit written notice through Blossom Medical Associates primary communication platform (e.g., Spruce) or via email 
  • Speak directly with a member of Blossom Medical Associates (phone or in person) 
  • Receive confirmation that the cancellation request has been reviewed and processed 

Requests submitted solely through text messaging outside of the approved platform, social media, or indirect communication channels will not be considered valid notice.


Medication-Based Fulfillment Requirement

Cancellation eligibility is based on both time and medication received.
Members must fulfill the financial obligation tied to the amount of medication dispensed before cancellation is effective.


No Refund Policy

All payments are non-refundable, including medication, services, and membership fees.


Medication Policy

All medications are final sale and cannot be returned or exchanged once dispensed or shipped.


Billing Disputes

By enrolling, members agree to contact the Blossom Medical Associates directly to resolve billing concerns prior to initiating any dispute.
